In 2020, five organizations — Meals on Wheels Diablo Region, Choice in Aging, Contra Costa Senior Legal Services, and Empowered Aging — came together to expand access to social services in East Contra Costa County. As the number of seniors in East County continues to grow, service providers must keep pace. This partnership reaches out to older adults in East Contra Costa, connecting them to the services they need.
Finding the services you need can be a confusing and frustrating process, but our Older Adult Support Specialist is here to help! This one-stop coordinated approach helps older adults receive the services they need to live a better, healthier life like:
- Home-delivered meals
- Adult day health programs
- Eviction protection
- Fall Prevention programs
- Exercise classes
- Companionship
- Care Management
- Legal services
- Elder Abuse prevention
- Advocacy for those in long-term care
- Much more!
Services are free and are available to anyone age 60 and older.

“I always thought I’d be ok when I got older. I joined the military right out of school and worked up until I got sick a few years back. Things got bad quickly and medical bills took everything I had in savings. It got to the point when I could only afford to eat cereal because my bills were so high. When unexpected emergencies arise we don’t want you choosing between eating and purchasing medicine, between paying your electric bill or paying rent. Often, our Care Management team can intervene and help. If you need one-time emergency assistance, call us. We may be of assistance.
Our goal is to help older adults remain healthy, happy, and independent for as long as possible. Occasionally, that means asking for help. If we don’t have a solution, we may be able to connect you with someone who does.

“When my husband started losing his memory, it became harder for me to take care of him on my own and we moved into our son’s house. I started noticing that my husband seemed afraid of our son, and I noticed the bruising on my dear husband’s arms. I was lost. How do you handle that?
